Baptist Health Plan provides a thorough process to address Member inquiries, complaints, grievances and appeals. These administrative remedies must be exhausted before legal remedies are sought.

Inquiries and Complaints

If you have concerns regarding medical treatment you have received, you should discuss your concerns with your healthcare Provider. If the problem is not resolved, or if the dissatisfaction is regarding your benefits, claims or concerns of another matter, you may contact a Baptist Health Plan Customer Service Representative at 859.269.4475 or 1.800.787.2680. Baptist Health Plan will respond to your complaint within fifteen (15) calendar days.

Grievances and/or Appeals

Baptist Health Plan’s grievance and appeal requirements are compliant with applicable state guidelines. Please select the appropriate state grievance and/or appeal process below in order to view the requirements for initiating a grievance or an appeal. Please Note: the applicable state is determined by the location of the policyholder (the subscriber’s employer group) and may not be your state of residency.
